Cortana understands me fine but VoiceAttack doesn't...

This is diving me mad... I really want to use VoiceAttack for Elite Dangerous but it rarely understands me.  However, if I click Cortana she understands me absolutely fine.

Examples:

If I say "Hello" Cortana hears "Hello.".  VoiceAttack hears "O".
If I say "Testing Testing 123" Cortana hears "testing testing 123". VoiceAttack hears nothing at all.
If I says "Banana" Cortana hears "Banana." VoiceAttack hears "clams".

Sometimes VoiceAttack shows the "Too Noisy" error, but if I record myself talking there is no background noise at all. It's really weird, it's like VoiceAttack is hearing something entirely different to windows.  Both are using the same mic source.

I have tried VoiceAttack using both the System Default speech engine and also Microsoft Speech Recognizer 8.0, but it doesn't appear to make any difference.

Am I missing something?
